Kindergarten immunizations available at Joplin Health Department

Now is the perfect time to get your child prepared for school!

• Joplin city Health Department will immunize children who reside within Joplin city limits, or who attend Joplin schools.

• We can issue birth certificates for anyone born in Missouri. Cost is $15 per certificate. We have forms for most other states if they were born outside of Missouri – the student will mail off paperwork and fees to obtain that birth certificate.

• Our walk-in clinics for “Kindergarten Round-up” will be 8:30 a.m. to 11:15 a.m. and 1 p.m. to 4 p.m. We will hold those clinics for the two weeks of March 21 to April 1. We do encourage appointments!

• Student must bring a shot record to receive immunizations. If they don’t have one, we recommend they contact their pediatrician to obtain one before they come to our clinic.

• Parent/guardian must bring a picture ID. If someone other than the guardian is bringing the child, they must also bring a note, signed by the parent/guardian, giving us permission to treat without a parent present.

• Costs for immunizations are as listed below:


VFC Eligible with Medicaid - will not have a Vaccine Charge, and Administration Charge billed to Medicaid

VFC Eligible no Medicaid - will not have a Vaccine Charge, and Administration Charge will be $10 per child

Insured with Partial Immunization Coverage - will have the Current vaccine cost and Administration Cost of $10 per child

Insured with Full Immunization Coverage - will be billed to Insurance through Vaxcare for both the vaccine cost and administration cost.

PLEASE CALL AHEAD AT 623-6122 TO DETERMINE WHICH OPTION WILL APPLY AND IF YOUR INSURANCE IS COVERED IN VAXCARE PROGRAM. APPOINTMENTS ARE APPRECIATED.
